Hardin County Job and Family Services is seeking a Service Coordinator/Kinship Navigator. Engage and promote partnerships between public and private agencies indluding schools, community, or faith-based organizations and relevant governmental agencies to increase the knowledge of the needs of kinship care and service coordination for families. Conduct crisis Helping Families stabilization and safety planning. Conduct successful research-based assessments with youth at different points during the Wraparound process to ensure positive outcomes. Create and coordinate implementation of plan of care. Serve as liaison between The State of Ohio Family and Children First and local Council and its subcommittees. Complete federal and state reports as required by ODJFS, Ohio FCFC and local funding sources. Completion of an Associate degree in Social Work orrelated field or secondary education or equivalent with four or more years of experience with high risk youth and families. Bach- elor's degree preferred. Service Coordination experience preferred.
